By submitting this form, you give The FQ permission to film, photograph, and/or record you via video, photography, and/or audio during the in-person, Zoom, LinkedIn Live, and/or Instagram Live session, and use the name, title, and photo you provide. You acknowledge that such recordings are the property of The FQ for purposes of publication, display or broadcast (print, web, digital display and all other forms of media). In addition, you understand and agree that all ownership and intellectual property rights related to the video and audio recorded by The FQ belong to The FQ.
